Can Iron Deficiency or Anaemia Give a False HbA1c Result? Diabetes Guide

Can anaemia cause a misleading HbA1c result? Yes. Iron deficiency, blood loss and other red blood cell disorders can sometimes make HbA1c disagree with actual glucose levels. Here is how to recognise and investigate the difference.

Can Iron Deficiency Cause a Falsely High HbA1c?

Iron deficiency anaemia may cause HbA1c to be higher than expected from actual glucose levels. The magnitude varies. HbA1c can change after iron treatment without a comparable change in blood sugar. This does not mean every elevated HbA1c is due to iron deficiency.

Can Anaemia Make HbA1c Falsely Low?

Yes. Haemolysis and recent blood loss shorten red blood cell survival and can lower HbA1c despite elevated glucose. Recent transfusion, advanced kidney disease, erythropoietin treatment and some haemoglobin variants can also complicate interpretation.

Why Does HbA1c Disagree With CGM or Glucometer Readings?

HbA1c reflects haemoglobin glycation over approximately 2–3 months. CGM measures interstitial glucose and provides a glucose management indicator (GMI), not a direct HbA1c measurement. Differences can arise from missed post-meal spikes, limited finger-stick testing, altered red cell turnover, haemoglobin variants or laboratory assay interference.

Conditions That Can Affect HbA1c

Condition Possible effect
Iron deficiency anaemia May falsely elevate HbA1c
Haemolysis or recent blood loss Often falsely lowers HbA1c
Recent transfusion Unpredictable
Haemoglobin variants Depends on assay and red cell survival
Advanced kidney disease May be misleading

Which Tests May Be Needed?

Your endocrinologist may review a complete blood count, ferritin and iron studies, kidney function, recent transfusion or blood loss, and the HbA1c laboratory method. CGM or structured finger-stick monitoring can help clarify true glucose exposure. Fructosamine or glycated albumin may be considered in selected cases, but these have their own limitations.

Should Diabetes Medicines Be Changed Based on HbA1c Alone?

No. When HbA1c and glucose readings disagree, investigate the discrepancy before intensifying or stopping treatment. Unnecessary intensification may cause hypoglycaemia, while a falsely low HbA1c may conceal hyperglycaemia.

Frequently Asked Questions

Can iron tablets change HbA1c?

Yes, correcting iron deficiency may change HbA1c independently of a major change in glucose. Review actual glucose measurements alongside repeat HbA1c.

Is HPLC always accurate in thalassaemia trait?

No. The impact of haemoglobin variants depends on the assay, and shortened red cell survival can affect results regardless of method.

Is CGM better than HbA1c?

They provide complementary information. CGM is particularly helpful when HbA1c is discordant with glucose patterns.
