GLP-1 medicines for belly fat and waist loss with semaglutide and tirzepatide, by Dr. Moxit Shah at Vishuddha Endocrine Clinic, Ahmedabad

Can GLP-1 Medicines Reduce Belly Fat? Semaglutide, Tirzepatide and Waist Loss

Medical author: Dr. Moxit Shah, DM Endocrinology | Vishuddha Endocrine Clinic, Ahmedabad

“Doctor, will this medicine reduce my belly?” It is one of the most common questions asked about semaglutide and tirzepatide. The answer is more encouraging—and more nuanced—than a simple yes or no.

Quick answer: GLP-1–based medicines can help eligible patients lose weight and reduce waist circumference, including abdominal fat as part of overall fat loss. They do not target belly fat alone. Waist measurement, metabolic health and preservation of muscle matter as much as the number on the scale.

Why belly fat matters more than the number on the scale

Two people can weigh the same but have very different fat distribution. Fat just beneath the skin is called subcutaneous fat. Fat stored deeper around the internal abdominal organs is called visceral fat. Excess visceral fat is associated with insulin resistance, type 2 diabetes, fatty liver and cardiovascular risk.

That is why a weight-loss consultation should consider waist circumference, blood pressure, glucose, lipids and overall health—not BMI alone. Waist size is a useful clinical marker but cannot directly measure the quantity of visceral fat.

Can semaglutide and tirzepatide reduce belly fat?

Yes, in appropriately selected patients, GLP-1–based treatment can reduce overall body fat and waist circumference. Semaglutide activates the GLP-1 receptor; tirzepatide activates both GIP and GLP-1 receptors. They help regulate appetite and food intake, supporting clinically meaningful weight reduction when combined with nutrition, movement and follow-up.

These medicines do not selectively “melt” belly fat. As weight and total adiposity decrease, abdominal size often decreases too. How much changes depends on initial body composition, treatment, dose, adherence, lifestyle and individual response.

What do clinical trials actually show?

In the STEP 1 trial involving adults with overweight or excess weight without diabetes, semaglutide 2.4 mg once weekly plus lifestyle intervention reduced average waist circumference by approximately 13.5 cm at 68 weeks, compared with approximately 4.1 cm with placebo plus lifestyle measures. These are group averages—not a prediction for any particular patient.

In the SURMOUNT-1 trial, tirzepatide plus lifestyle intervention reduced average waist circumference by approximately 14.0 cm, 17.7 cm and 18.5 cm at 72 weeks across the 5 mg, 10 mg and 15 mg groups respectively, compared with approximately 4.0 cm with placebo. This study excluded participants with diabetes. Results from one trial should not be treated as a head-to-head comparison with another trial.

Study populations, treatment durations, doses and definitions matter. Trial results do not guarantee an individual outcome, and access and approvals differ by country.

Why waist circumference is especially relevant in India

People of South Asian ancestry may develop metabolic complications at lower BMI values than some other populations. For South Asian adults, commonly used waist-risk thresholds are approximately 90 cm or more in men and 80 cm or more in women. These are screening thresholds for central adiposity and risk assessment, not automatic eligibility criteria for GLP-1 treatment.

Even with a BMI that does not look particularly high, an increased waistline can prompt a discussion about metabolic health. Conversely, one waist measurement is not enough to diagnose a disease or decide on medicine.

How to measure your waist correctly at home

  1. Stand upright with your feet roughly hip-width apart and your abdomen relaxed.
  2. Use a flexible tape measure at a consistent anatomical landmark, preferably midway between the lower rib and the top of the hip bone.
  3. Keep the tape horizontal and snug without pressing into the skin.
  4. Measure after gently breathing out, not while holding your breath.
  5. Record the date and measurement in centimetres; compare under similar conditions every two to four weeks.

Daily waist measurements are usually unnecessary. For many patients, tracking waist size along with weight, strength and metabolic markers is more useful than focusing only on weekly kilograms lost.

Can you lose centimetres without losing many kilograms?

Sometimes waist measurements change even when the scale moves slowly. Differences in bloating, hydration, muscle mass, body composition and measurement technique can contribute. Regular strength exercise may help preserve muscle during weight reduction, but a shrinking waist does not prove that only visceral fat has been lost.

If weight loss seems to have stalled, review trends across several weeks rather than reacting to one reading. See our GLP-1 weight-loss plateau guide.

How to support healthy weight loss while taking GLP-1 medicines

1. Protect your protein intake

When appetite is reduced, avoid living on tea, biscuits or just a few bites. Include suitable protein sources such as dal, chickpeas, paneer, curd, eggs, tofu or other foods that fit your preferences and medical conditions. Protein recommendations should be individualized, especially in advanced kidney disease.

2. Maintain muscle through activity

Walking is helpful, but resistance exercises suited to your capacity can help protect strength and function during weight loss. Start gradually and ask for guidance if you have heart, joint or balance limitations.

3. Build balanced Indian meals

A practical meal may include sabzi and salad, dal or paneer, and a measured portion of roti or rice. Do not assume millet, brown rice or “diet” foods are carbohydrate-free. For detailed meal guidance, see what to eat while taking GLP-1 medicines.

4. Stay hydrated and manage side effects

Nausea, vomiting, constipation or diarrhoea can interfere with eating and drinking. Seek medical advice for persistent symptoms and urgent care for severe abdominal pain, repeated vomiting or signs of dehydration. Do not escalate or alter doses independently.

Who should consider GLP-1 treatment for weight loss?

GLP-1–based medicines are prescription treatments and are not intended for cosmetic spot reduction or quick weight loss before an event. Treatment eligibility depends on the product’s approved indication, BMI and weight-related health conditions, past medical history, pregnancy plans, other medicines and an individualized risk-benefit discussion. Your clinician should check the locally approved Indian product information.

Important safety considerations can include pregnancy, pancreatitis and gallbladder symptoms, severe gastrointestinal intolerance, and personal or family history of medullary thyroid carcinoma or MEN2 for products carrying those contraindications. These medicines may also require adjustment of other glucose-lowering drugs to avoid hypoglycaemia.

What else should your doctor track besides waist size?

  • Body weight trend: over weeks and months, not day to day.
  • Waist circumference: measured consistently.
  • Strength and function: energy, exercise performance and muscle preservation.
  • Blood pressure, HbA1c and lipids: depending on your condition.
  • Tolerability and nutrition: adequate meals, hydration and side-effect management.
  • Long-term treatment plan: maintaining improvements after initial weight loss.

Frequently asked questions

Does Ozempic or Wegovy target belly fat specifically?

No. Semaglutide supports overall weight reduction and often reduces waist circumference. It does not selectively remove fat from one body area.

Does Mounjaro reduce waist circumference?

Clinical trials of tirzepatide have shown substantial average reductions in waist circumference. Individual results vary.

How quickly will my waist get smaller?

There is no guaranteed timeline. Many treatments begin with gradual dose increases, and responses differ. Judge progress over several weeks and months rather than expecting rapid changes.

What is a healthy waist size for Indian adults?

Risk-screening thresholds commonly used for South Asian adults are 90 cm for men and 80 cm for women. Your personal risk and treatment plan depend on much more than this number.

Can someone with normal BMI take GLP-1 solely for belly fat?

These are not routine treatments for isolated cosmetic abdominal fat. Eligibility should be assessed against an approved medical indication and overall risk.

Does belly fat return after stopping GLP-1 treatment?

Weight regain is possible after treatment withdrawal, so an ongoing lifestyle and medical follow-up plan matters. Read more about weight regain after stopping GLP-1 medicines.
