Afrezza: New Inhaled Insulin Without Mealtime Injections — How Does It Work?
By Dr. Moxit Shah, DM Endocrinology | Vishuddha Endocrine Clinic, Ahmedabad
Afrezza is a rapid-acting human insulin powder inhaled through the mouth into the lungs using a dedicated inhaler. It helps control blood sugar rises around meals. It is not an oral tablet, an asthma inhaler or a substitute for every insulin injection.
Is Afrezza Available in India?
Cipla announced the launch of Afrezza in India on 22 December 2025, following CDSCO approval for distribution and marketing. Actual stock, price and the locally approved prescribing instructions should be confirmed with an authorised pharmacy and clinician. The India launch does not mean every pharmacy in Ahmedabad has it in stock.
How Does Afrezza Inhaled Insulin Work?
Afrezza contains dry-powder human insulin in single-use cartridges. After inhalation, insulin is absorbed through the lungs into the bloodstream and acts rapidly. It is taken at the beginning of meals as prescribed to manage post-meal glucose rises. The inhaler delivers insulin; it does not treat respiratory illness.
Afrezza vs Injectable Mealtime Insulin
| Feature | Afrezza | Injected rapid-acting insulin |
|---|---|---|
| Delivery | Inhaled through mouth | Subcutaneous injection |
| Purpose | Mealtime blood sugar | Mealtime blood sugar |
| Timing | Beginning of meal | Depends on formulation |
| Special monitoring | Baseline and follow-up spirometry | Not routinely required solely for insulin |
| Type 1 Diabetes | Must be used with basal insulin | Basal insulin also needed |
Who Can Use Afrezza?
Suitability depends on Diabetes type, age, lung health, smoking history, glucose patterns, ability to use the device and local prescribing information. The current US label includes adults and children aged six years and older. Indian patients should follow the locally approved age indication, which should not be assumed to match the US label.
Can Afrezza Replace Insulin Injections in Type 1 Diabetes?
No. People with Type 1 Diabetes must continue basal insulin, generally given by injection or pump. Afrezza may serve as a mealtime option in selected patients but is not suitable for treating diabetic ketoacidosis (DKA). It cannot replace the entire insulin regimen.
Why Is a Lung Function Test Necessary?
Afrezza carries a boxed warning about sudden bronchospasm in people with chronic lung disease. Before starting, the US label requires medical history, examination and spirometry (FEV1). It recommends repeating lung function testing at six months and annually thereafter. Follow the Indian label and specialist assessment for local prescribing.
Who Should Not Use Afrezza?
- Patients with asthma or COPD: contraindicated because of bronchospasm risk.
- Patients experiencing hypoglycaemia.
- Patients with serious hypersensitivity to insulin or product components.
- People who currently smoke or recently stopped: use is not recommended.
- Patients with DKA: this is not an appropriate DKA treatment.
Benefits, Side Effects and Limitations
Afrezza may reduce the need for mealtime injections in selected patients and has a rapid onset. It can still cause hypoglycaemia, cough, throat irritation and changes in lung function. DKA has been reported more frequently in some Type 1 Diabetes trials. Afrezza cartridge units are not directly interchangeable with units of injected insulin: a clinician must use product-specific conversion guidance and glucose monitoring.
How Is Afrezza Used?
- Have a Diabetes specialist confirm suitability and review spirometry.
- Learn the correct inhaler technique and cartridge handling.
- Use the prescribed cartridge at the beginning of the meal, according to the treatment plan.
- Monitor glucose or CGM readings closely when starting or changing treatment.
- Continue prescribed basal insulin and keep an illness and hypoglycaemia action plan.
Afrezza Price and Availability in Ahmedabad
Afrezza price in India (October 2026): Indian pharmacy listings show the following MRPs for packs of 30 single-use cartridges with an inhaler. Discounted selling prices may be lower, and prices and availability vary by pharmacy and location.

| 4 units | 30 cartridges + inhaler | ₹3,999 | ₹133 |
| 8 units | 30 cartridges + inhaler | ₹4,599 | ₹153 |

How much does Afrezza cost per month? For illustration only, using one cartridge with each of three daily meals requires about 90 cartridges or three 30-cartridge packs per 30 days. At listed MRPs, this would cost approximately ₹11,997/month for 4-unit cartridges, ₹13,797/month for 8-unit cartridges or ₹16,317/month for 12-unit cartridges. These are examples, not recommended doses. A patient may need a different number or combination of cartridges, and the actual monthly cost may be substantially different. Additional costs may include spirometry and clinical follow-up.

Afrezza has launched in India, but local pharmacy stock and current pack prices have not been independently verified here. Ask an authorised pharmacy for the available strengths, monthly cost and supply. Factor in the cost of spirometry and follow-up monitoring. Avoid buying insulin from unverified online sellers.
Frequently Asked Questions
Is Afrezza completely needle-free?
Afrezza avoids injections for the mealtime doses it replaces, but patients may still need basal insulin injections or a pump, especially with Type 1 Diabetes.
Can someone with asthma use Afrezza?
No. Afrezza is contraindicated in patients with asthma or COPD because it may cause acute bronchospasm.
Can Afrezza be used instead of insulin during DKA?
No. DKA is a medical emergency and Afrezza is not recommended for its treatment.
Can I switch from injected insulin to Afrezza myself?
No. The dosing conversion is not one-to-one. Switching requires specialist guidance, monitoring and a safety plan.
Is Afrezza available in Ahmedabad?
It has been launched in India, but availability at a specific Ahmedabad pharmacy needs confirmation.
