Diabetes care is no longer focused only on bringing HbA1c down. Modern treatment also considers cardiovascular disease, kidney disease, heart failure, weight, hypoglycemia risk and the person’s preferences.

Why HbA1c Is Not the Whole Story

HbA1c is an important measure of average glucose, but two people with the same HbA1c may have very different cardiovascular and kidney risks. Treatment therefore needs to consider the full clinical picture.

When Cardiovascular and Kidney Risk Changes Treatment

ADA 2026 recommends that people with type 2 diabetes and established cardiovascular disease or chronic kidney disease receive appropriate glucose-lowering therapy with demonstrated cardiovascular benefit, including selected GLP-1 receptor agonists and/or SGLT2 inhibitors. For heart failure, an SGLT2 inhibitor with proven benefit is recommended in appropriate patients.

What About People Without Established Heart Disease?

Risk assessment still matters. Blood pressure, lipids, smoking, kidney function, weight and family history contribute to long-term cardiovascular risk. Treatment choices can be individualized around these factors.

Lifestyle Remains Essential

  • Choose a sustainable, nutrient-dense eating pattern.
  • Exercise regularly and reduce prolonged sitting.
  • Work toward a healthy weight when appropriate.
  • Do not smoke.
  • Sleep adequately and address stress.

Why Medication Choice Should Be Individualized

ADA 2026 recommends shared decision-making that considers cardiovascular, kidney and weight effects, hypoglycemia risk, adverse effects, cost, access and patient preferences. Therefore, the “best” diabetes medicine is not identical for every person.

Bottom Line

Good diabetes management means protecting the heart, kidneys, eyes and nerves—not simply chasing a lower HbA1c. A personalized plan can combine lifestyle measures with the medicines that provide the most relevant benefits for that individual.
