Afrezza Needle-Free Insulin: Can You Avoid Mealtime Injections?
By Dr. Moxit Shah, DM Endocrinology | Vishuddha Diabetes, Thyroid, Weight Loss Clinic, Ahmedabad
Can people with diabetes take insulin without mealtime injections? For some patients, yes. Afrezza is a rapid-acting inhaled insulin that may replace selected mealtime injections, but it is not appropriate for everyone and does not replace basal insulin when required.
What is Afrezza inhaled insulin?
Afrezza is rapid-acting human insulin delivered as a dry powder through a specially designed inhaler. Instead of a mealtime injection, a prescribed cartridge is placed in the device and inhaled at the beginning of a meal. It treats meal-related glucose rises; it is not a replacement for every form of insulin.
How does needle-free insulin work?
The powder reaches the lungs, where insulin is absorbed rapidly into the bloodstream. Insulin helps the body use glucose and reduces post-meal blood sugar. Afrezza acts rapidly and has a shorter duration of glucose-lowering effect than many injected rapid-acting insulin analogues. Its dosing and glucose effects require individualized monitoring.
Can Afrezza replace all insulin injections?
No. People with type 1 diabetes must also use basal insulin. Some adults with type 2 diabetes may use Afrezza for meals as part of an individualized regimen. Afrezza cannot replace background insulin when it is required, and it is not recommended for treating diabetic ketoacidosis.
Afrezza vs NovoRapid and Humalog
| Feature | Afrezza | NovoRapid / Humalog |
|---|---|---|
| Delivery | Inhaled powder | Subcutaneous injection |
| Purpose | Mealtime insulin | Mealtime insulin |
| Needle | No for this dose | Yes |
| Action | Very rapid, shorter duration | Rapid, generally longer duration |
| Lung-function assessment | Required | Not routinely required |
Cartridge units are not directly interchangeable with injected insulin units. Dose conversion must be supervised. See our guide to switching from NovoRapid or Humalog to Afrezza.
Who might benefit from Afrezza?
Suitable patients who need mealtime insulin and prefer to avoid repeated mealtime injections may consider Afrezza. It may also suit selected people with troublesome post-meal glucose excursions. The decision must account for glucose patterns, insulin needs, lung health, availability and affordability. Being needle-free alone does not make it the best choice for everyone.
Who should not use Afrezza?
Afrezza is contraindicated in chronic lung disease such as asthma or COPD, during hypoglycaemia and in patients with hypersensitivity to human insulin or product ingredients. It is not recommended in people who smoke or recently stopped smoking. People with a history of lung cancer or at increased risk require particular caution. It should not be used for diabetic ketoacidosis.
Why is spirometry required?
Inhaled insulin can affect lung function and may cause acute bronchospasm. The US prescribing information recommends lung-function assessment with spirometry before starting, at approximately six months, and annually thereafter even without respiratory symptoms. Persistent cough, wheeze or breathlessness requires review. Significant decline in FEV1 may prompt discontinuation.
What side effects should patients know?
Hypoglycaemia remains a major insulin-related risk. Cough and throat irritation can occur. Serious risks include acute bronchospasm, changes in lung function, severe allergic reactions and hypokalaemia. Patients should know how to recognize and promptly treat low blood sugar and when to seek urgent care for breathing difficulty.
How is Afrezza taken?
Afrezza uses single-use insulin cartridges and a compatible inhaler. The US formulation has 4-, 8- and 12-unit cartridges. Patients should be trained on cartridge loading and inhalation technique, and should take it at the beginning of a meal as prescribed. Do not swallow the powder or alter cartridge combinations without instructions. Glucose monitoring is essential when starting or changing the regimen.
Is Afrezza available in India?
Afrezza received Indian regulatory approval for adults with diabetes in December 2024, and Cipla announced its commercial launch in India in December 2025. Specific pharmacy availability and pricing vary and should be checked locally. US labeling expanded to pediatric patients aged six years and older in May 2026; that US indication does not automatically establish pediatric approval in India.
Can Afrezza improve HbA1c?
Appropriately prescribed mealtime insulin can help reduce glucose excursions and HbA1c, but results depend on baseline glucose, background treatment, food intake, dosing and monitoring. Afrezza should not be assumed to be universally superior to injected insulin. See our guide to HbA1c targets and CGM time-in-range guide.
Checklist before starting needle-free insulin
- Confirm that mealtime insulin is needed.
- Review asthma, COPD, smoking and other respiratory history.
- Complete baseline spirometry.
- Discuss whether basal insulin must continue.
- Learn correct inhaler technique and individualized dose conversion.
- Keep a glucose-monitoring and hypoglycaemia action plan.
- Check cartridge availability, price and follow-up arrangements.
Frequently asked questions
Is Afrezza completely needle-free?
The mealtime dose is inhaled, but patients needing basal insulin may still require injections.
Can Afrezza replace NovoRapid or Humalog?
Sometimes, with clinician-directed conversion and monitoring. Do not substitute units one-for-one.
Can people with type 1 diabetes use it?
Where approved and suitable, yes, alongside basal insulin.
Is Afrezza safe for people with asthma?
No. Asthma and COPD are contraindications because of bronchospasm risk.
Does Afrezza cause hypoglycaemia?
Yes, like other insulin products.
Is Afrezza available in Ahmedabad?
It has been commercially launched in India, but individual pharmacy stock should be confirmed.
