How to Switch From NovoRapid or Humalog to Afrezza: Start With One Meal, Then Progress Safely

By Dr. Moxit Shah, DM Endocrinology | Vishuddha Endocrine Clinic, Ahmedabad

Afrezza is an ultra-rapid inhaled mealtime insulin that may replace NovoRapid (insulin aspart) or Humalog (insulin lispro) injections for selected people with Diabetes. Switching is not a unit-for-unit substitution. A practical, clinician-supervised transition can begin with one selected meal, assess the glucose response and inhaler technique, and then expand to other meals when safe. The one-meal-first method is a pragmatic individualised strategy, not a requirement specified by the US Afrezza label.

Why Consider Switching From NovoRapid or Humalog to Afrezza?

NovoRapid and Humalog are injected rapid-acting insulin analogues. Afrezza is inhaled human insulin taken at the beginning of a meal. It acts quickly and has a shorter glucose-lowering tail than many injected mealtime analogues. Some patients value avoiding multiple mealtime injections, but insulin absorption, dose needs and glucose patterns differ. Patients with Type 1 Diabetes must continue appropriate basal insulin.

Step 1: Check Whether Afrezza Is Safe

  • Review the patient’s Diabetes type, meal pattern, current basal-bolus doses, glucose logs or CGM, and history of hypoglycaemia or ketoacidosis.
  • Screen for asthma, COPD and other chronic lung disease; Afrezza is contraindicated in asthma and COPD due to risk of acute bronchospasm.
  • Perform baseline spirometry including FEV1 before treatment; repeat after 6 months and annually, or sooner if respiratory symptoms develop.
  • It is not recommended for people who smoke or recently stopped smoking, and it is not a treatment for diabetic ketoacidosis.
  • Review ability to use the inhaler correctly, pregnancy considerations, affordability, cartridge supply and locally applicable prescribing information.

Step 2: Convert the NovoRapid or Humalog Mealtime Dose

The current US Afrezza prescribing information gives the following starting conversion table for a single injected mealtime dose. This is a starting point only; follow-up titration is necessary. Afrezza cartridge units are not bioequivalent to injected insulin units.

Current NovoRapid / Humalog dose at one mealUS-label starting Afrezza dose at that meal
Up to 3 units4 units inhaled
4–5 units8 units inhaled
6–7 units12 units inhaled
8 units or more16 units inhaled initially; reassess and titrate

For doses exceeding one cartridge, combinations of 4-, 8- and 12-unit cartridges may be used. Do not automatically interpret the final row as an adequate maintenance dose for a person previously using a much larger injection dose. Assess glucose response and titrate individually with the prescribing clinician.

Step 3: Consider a Supervised One-Meal Trial First

For a stable, appropriately screened patient, the treating endocrinologist may elect to switch one meal first, such as lunch, while retaining the usual prescribed NovoRapid or Humalog at the other meals. This staged approach can help evaluate inhaler technique, meal coverage, post-meal glucose and tolerance before expanding treatment. It is an optional clinical approach, not an FDA-mandated sequence. Never use Afrezza and the former injected bolus for the same meal unless the prescriber has specifically instructed a correction strategy.

Example: Transition at Lunch

Suppose a patient currently uses NovoRapid before breakfast, lunch and dinner. After lung screening and an individualised dose calculation, the clinician may replace the lunch injection with Afrezza at the beginning of lunch, while leaving breakfast and dinner injection plans unchanged. Continue prescribed basal insulin. Check glucose before the meal and monitor the post-meal response closely, ideally with CGM or structured finger-stick readings. If control or tolerance is unsatisfactory, reassess rather than automatically proceeding to more meals.

Step 4: Review Glucose Data and Adjust

Review pre-meal glucose, approximately 1–2-hour post-meal readings or CGM trends, late post-meal highs, hypoglycaemia, meal carbohydrate content and inhalation technique. Because Afrezza has a shorter action profile, later hyperglycaemia can occur after higher-fat or prolonged meals. Any extra dosing or correction should follow a clinician-defined plan; avoid insulin stacking and unplanned repeat inhalations. Review promptly if persistent hyperglycaemia, ketones, respiratory symptoms or recurrent hypoglycaemia occur.

Step 5: Expand From One Meal to Two, Then Three if Appropriate

If the initial selected meal shows acceptable glucose coverage, safe technique and good tolerance, the clinician may consider replacing a second mealtime injection, and subsequently the third. Each meal requires its own conversion and glucose review; breakfast, lunch and dinner doses need not be identical. There is no universal number of days to wait between meals. Some patients may remain on Afrezza for only one or two meals, or return to injections if it does not provide adequate control.

Practical Monitoring Checklist

  • Before switch: baseline spirometry, inhaler training, basal insulin review, CGM or SMBG plan, hypoglycaemia education.
  • During one-meal trial: glucose before and after the selected meal, symptoms of low glucose, inhaler technique, cough or wheeze.
  • Before adding another meal: confirm acceptable post-meal coverage and a documented titration plan.
  • Longer term: HbA1c/CGM review, FEV1 at 6 months and annually, treatment satisfaction and cost.

Who Should Not Switch Without Specialist Assessment?

People with asthma or COPD should not use Afrezza. Additional caution is needed with unstable Type 1 Diabetes, frequent DKA, severe or unexplained hypoglycaemia, significant respiratory symptoms, smoking, or unreliable glucose monitoring. Afrezza does not replace basal insulin and should not be used to treat DKA.

Afrezza Price and Availability in India

Verify current stock, cartridge strength, inhaler supply and prescription requirements with an authorised pharmacy. The total monthly cost depends on actual cartridges used per meal and any prescribed adjustments.

Frequently Asked Questions

Can I stop NovoRapid or Humalog completely on day one?

Some treatment plans may replace all mealtime doses under close supervision, but a one-meal-first transition is a reasonable option for selected stable patients. Do not stop or change prescribed insulin on your own.

Is 8 units of Afrezza the same as 8 units of Humalog?

No. The insulin products have different pharmacokinetic and pharmacodynamic profiles. Use the product-specific starting conversion guidance and individual titration.

Can patients with Type 1 Diabetes use Afrezza?

Selected patients may use Afrezza for meals, but must continue an appropriate basal insulin regimen. Watch closely for hyperglycaemia and ketosis.

Does Afrezza eliminate every injection?

Not necessarily. People who require injected basal insulin will still need that treatment.
