A thyroid blood test is one of the most useful ways to assess whether your thyroid is working normally. The most commonly used tests are TSH, Free T4 (FT4), and sometimes T3. Understanding what each test measures can make your report much easier to understand.
What is a thyroid blood test?
Thyroid function testing usually starts with a blood test for TSH. Depending on the clinical situation, your doctor may add Free T4, T3, thyroid antibodies, or other tests. Test results should always be interpreted with your symptoms, medications, pregnancy status and the laboratory’s own reference range.
What is TSH?
TSH (thyroid-stimulating hormone) is produced by the pituitary gland and regulates thyroid hormone production. TSH is generally the first blood test used to assess thyroid function. A high TSH commonly suggests an underactive thyroid, while a low TSH commonly suggests an overactive thyroid, although there are important exceptions such as pituitary disease.
What is Free T4?
Free T4 (FT4) measures the unbound fraction of thyroxine available to tissues. When interpreted together with TSH, FT4 helps distinguish patterns such as overt hypothyroidism from subclinical hypothyroidism and helps assess hyperthyroidism.
When is T3 tested?
T3 is particularly useful when hyperthyroidism is suspected. In some people with a low TSH, T3 may be elevated even when FT4 is still normal. T3 is usually less useful for diagnosing hypothyroidism because it may remain normal even when a person has significant hypothyroidism.

Do you need fasting for a thyroid blood test?
For routine thyroid function testing, fasting is generally not required. However, if your doctor has ordered other blood tests at the same time, those tests may require fasting. Follow the preparation instructions provided by your laboratory or doctor.
Should you take your thyroid tablet before the blood test?
If you take levothyroxine or another thyroid medicine, do not change or stop your treatment on your own. The timing of the dose and blood sample can matter in some situations, so follow the instructions given by your treating doctor, especially when a dose adjustment is being assessed.
Can biotin affect thyroid test results?
Yes. High-dose biotin, commonly found in some hair, skin and nail supplements, can interfere with certain thyroid laboratory assays and produce misleading results. The American Thyroid Association advises avoiding biotin for at least 2 days before thyroid blood testing; tell your doctor about supplements you take so the appropriate advice can be given.

Are thyroid antibody tests always necessary?
No. Thyroid antibodies such as anti-TPO antibodies or TSH-receptor antibodies are ordered when the clinical question requires them—for example, when an autoimmune thyroid disorder is suspected. They are not automatically needed with every routine thyroid blood test.
How often should thyroid tests be repeated?
The interval depends on why the test was ordered. People starting or changing thyroid hormone treatment may need earlier repeat testing, while stable patients may need less frequent monitoring. Your endocrinologist can choose the interval based on the diagnosis, medication dose and clinical situation.
When should you see an endocrinologist?
Consider specialist review when thyroid results are persistently abnormal, results do not fit your symptoms, there is pregnancy or pre-pregnancy planning, a thyroid nodule or goitre is present, or treatment decisions are complex.

Is TSH enough to diagnose a thyroid problem?
TSH is an excellent starting test, but it is not sufficient in every clinical situation. FT4, T3, antibody tests or other investigations may be appropriate depending on the suspected disorder.
What is the normal TSH range?
There is no single universal TSH range that applies to every person and every laboratory. Use the reference interval printed on your report and interpret it in clinical context.
Can thyroid symptoms be diagnosed from symptoms alone?
No. Symptoms such as tiredness, weight change, palpitations or changes in bowel habits can have many causes. Blood testing helps establish whether thyroid dysfunction is present.
