Levothyroxine is one of the most commonly prescribed medicines in India. Millions of people with Hypothyroidism take a small thyroid tablet every morning, often for many years.
But how you take Levothyroxine matters almost as much as the dose.
Taking it with tea, with breakfast, or together with Calcium or Iron tablets can reduce how much of the medicine your body absorbs. The result is often a TSH report that keeps changing, and a dose that keeps being adjusted, when the real problem is the timing.

What is Levothyroxine and why is it prescribed?
Levothyroxine is a synthetic form of thyroxine (T4), the main hormone made by the thyroid gland. It is prescribed when the thyroid does not make enough hormone, most often because of Hashimoto’s Thyroiditis, after thyroid surgery, or after radioactive iodine treatment.
In India it is sold under brand names such as Thyronorm, Eltroxin and Thyrox. The medicine inside is the same: Levothyroxine sodium.
Because the body absorbs it from the small intestine, anything that sits in the stomach at the same time can interfere with it.
What is the best time to take Levothyroxine?
For most people: in the morning, on an empty stomach, with a glass of plain water, 30 to 60 minutes before breakfast or tea.

Several common medicines reduce absorption when taken close to the thyroid tablet. Keep a gap of about 4 hours for:
- Calcium tablets
- Iron tablets, including many pregnancy supplements
- Multivitamins with minerals
- Antacids containing aluminium or magnesium
- Sucralfate
- Cholesterol-binding resins such as Cholestyramine
Other medicines can affect your thyroid levels in other ways, including acid-reducing medicines such as Pantoprazole and Omeprazole, some epilepsy medicines, and oral contraceptive pills or hormone therapy.
You do not need to stop these medicines. Tell your doctor about everything you take, so the timing and dose can be planned.
Do foods affect Levothyroxine?
Yes. Food in the stomach generally reduces absorption, which is why the empty-stomach rule matters. Soy products and very high-fibre breakfasts can also reduce absorption.
You do not need to give up these foods. Keep your breakfast pattern reasonably regular, so your dose can be matched to your routine.
What if I miss a dose of Levothyroxine?
Levothyroxine stays in the body for about a week, so missing a single dose is not dangerous. Take it when you remember that day, following the same empty-stomach rule.

Can I switch brands of Levothyroxine?
Different brands contain the same medicine, but small differences between products can change your TSH in some people.
It is best to stay on the same brand. If you do change brands, tell your doctor, and your thyroid test may be repeated after about 6 to 8 weeks.
When should I recheck my thyroid test?
TSH changes slowly. After starting Levothyroxine or changing the dose, it is usually rechecked after about 6 to 8 weeks. Once the dose is stable, testing is often done every 6 to 12 months.
Tell your doctor or lab if you take Biotin, which is present in many hair and nail supplements. It can falsely change thyroid test results, and you may be asked to stop it for a few days before the test.
