More women of reproductive age are now taking GLP-1 medicines such as Semaglutide (Ozempic, Wegovy) and Tirzepatide (Mounjaro), for Obesity, Type 2 Diabetes or PCOS.
That makes pregnancy planning an essential conversation.
These medicines are not recommended during pregnancy, some need to be stopped weeks before trying to conceive, and Tirzepatide can make oral contraceptive pills less effective. This guide explains what women on GLP-1 treatment should know.
Can I take GLP-1 medicines during pregnancy?
No.
GLP-1 medicines are not recommended during pregnancy. There is limited safety data in human pregnancy, and weight loss is not an appropriate goal while pregnant. WHO’s obesity guideline on GLP-1 therapies also excludes pregnant women.
When should I stop before trying to conceive?
Weekly GLP-1 medicines stay in the body for a long time.
- Semaglutide: the prescribing information advises stopping at least 2 months before a planned pregnancy
- Tirzepatide: also long-acting. Plan the timing of stopping with your doctor before trying to conceive
The key is planning. Tell your doctor as soon as you start thinking about pregnancy, not after.
Does Tirzepatide affect the contraceptive pill?
Yes. This is important and often missed.
Tirzepatide slows stomach emptying and can reduce how well oral contraceptive pills are absorbed. The prescribing information advises women using oral contraceptives to switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting Tirzepatide and for 4 weeks after each dose increase.
Non-oral options, such as an IUD, implant or injection, are not affected in the same way. Vomiting or diarrhea from any cause can also reduce pill effectiveness.
Can GLP-1 medicines improve fertility?
Weight loss can improve ovulation, particularly in women with PCOS. Some women become pregnant unexpectedly after starting treatment.
This is another reason to have reliable contraception if pregnancy is not planned. Read about GLP-1 medicines and PCOS.
What if I become pregnant while taking a GLP-1 medicine?
- Stop the medicine
- Contact your doctor promptly
- Do not panic. Many women in this situation have healthy pregnancies, and your doctor will guide monitoring
- Start Folic Acid if you have not already
What about Diabetes control before pregnancy?
For women with Type 2 Diabetes, good glucose control before conception reduces the risk of miscarriage and birth defects.
When the GLP-1 medicine is stopped, blood sugar may rise, so another plan is needed, often Insulin, which is safe in pregnancy. The ADA suggests an HbA1c below 6.5% before conception, if this can be achieved without Hypoglycemia.
See Diabetes during pregnancy.
Will I regain weight after stopping?
Some weight regain is common after stopping GLP-1 medicines. Before stopping, build habits that help: protein-rich meals, regular activity and resistance exercise.

What about breastfeeding?
Data on GLP-1 medicines during breastfeeding are limited, and they are generally not recommended while breastfeeding. Discuss the timing of restarting treatment with your doctor after delivery.
A pre-pregnancy checklist
- Tell your doctor about pregnancy plans early
- Plan when to stop the GLP-1 medicine
- Use effective contraception until then
- Start Folic Acid before conception
- Optimise blood sugar, thyroid and blood pressure
- Review all other medicines for pregnancy safety

Frequently Asked Questions About GLP-1 and Pregnancy
How long before pregnancy should I stop Ozempic or Wegovy?
At least 2 months before a planned pregnancy, as advised in the prescribing information.
Does Mounjaro stop the pill from working?
It can reduce its effectiveness, especially after starting and after dose increases. Use a non-oral or barrier method during those times.
Do GLP-1 medicines cause infertility?
There is no evidence of this. Weight loss often improves fertility.
Can I use GLP-1 medicines for PCOS if I want pregnancy later?
Sometimes, as part of a planned approach with a clear stopping time before conception.
I got pregnant on Semaglutide. What now?
Stop the medicine and contact your doctor promptly for guidance.
Final Takeaway
GLP-1 medicines and pregnancy need planning, not guesswork.
Early discussion + reliable contraception + backup with Tirzepatide + planned stopping before conception + Folic Acid + glucose optimisation + follow-up.
If you are on GLP-1 treatment and considering pregnancy, consult an endocrinologist in advance.
