A high prolactin level, also called hyperprolactinemia, means that the blood level of prolactin is higher than expected. Prolactin is produced mainly by the pituitary gland and is best known for its role in breast milk production. Outside pregnancy and breastfeeding, an elevated prolactin level may affect periods, fertility, sexual function and bone health.
What Causes High Prolactin?
High prolactin does not always mean that there is a pituitary tumor. Common causes include pregnancy and breastfeeding, certain medicines, hypothyroidism, kidney or liver disease, stress and disorders affecting the pituitary region. A prolactin-secreting pituitary tumor, called a prolactinoma, is another important cause.
Medicines that can raise prolactin include some antipsychotic medicines, medicines used for nausea and vomiting, certain antidepressants, opioids and some blood-pressure medicines. Do not stop a prescribed medicine on your own; the medication list should be reviewed with your doctor.
Symptoms of High Prolactin

The evaluation may include a pregnancy test when appropriate, thyroid function tests, review of medicines and assessment for other pituitary hormone abnormalities. In selected patients, testing for macroprolactin can help identify an inactive form of prolactin. If a pituitary cause is suspected, an MRI of the pituitary may be recommended.
Does Every High Prolactin Level Need Treatment?
No. Treatment depends on the cause, the degree of elevation, symptoms, reproductive goals and whether a pituitary lesion is present. Some people with an elevated prolactin level and no clinically important symptoms may only need observation and appropriate evaluation.
How Is Hyperprolactinemia Treated?
Treatment is directed at the underlying cause. If a medicine is responsible, the prescribing doctor may consider an alternative when appropriate. If hypothyroidism is the cause, thyroid hormone replacement can correct the thyroid disorder and may normalize prolactin.
For prolactinomas, dopamine agonists such as cabergoline or bromocriptine are commonly used. They can lower prolactin and, in many patients, reduce tumor size. Surgery is considered in selected situations, such as when medical treatment is ineffective or not tolerated.
