Missed weekly semaglutide or tirzepatide dose: safe next steps and dose timing guide from Dr. Moxit Shah, Ahmedabad

Missed a Semaglutide or Tirzepatide Dose? What to Do Safely

By Dr. Moxit Shah, DM Endocrinology | Vishuddha Diabetes, Thyroid, Weight Loss Clinic, Ahmedabad

Missing a weekly Weight Loss or diabetes injection is common, but the safe next step depends on which medicine you take. This guide explains what the results or treatment instructions mean and when medical review is appropriate.

Why missed-dose rules differ

Semaglutide and tirzepatide are not interchangeable, and the correct advice depends on the brand, indication, formulation and local product label. This guide covers commonly used once-weekly injectable products. Always check the patient leaflet supplied with your specific medicine.

Missed weekly semaglutide (Ozempic)

For Ozempic, commonly used prescribing information says to take the missed dose as soon as possible within 5 days of the scheduled dose. If more than 5 days have passed, skip that dose and resume on your usual scheduled day. Do not take two doses to compensate. Follow your country’s current label.

Missed weekly semaglutide (Wegovy)

Wegovy instructions differ from Ozempic. For weekly Wegovy, take the missed dose if the next scheduled dose is more than 48 hours away; otherwise skip and resume your regular schedule. If two or more consecutive doses are missed, contact the prescriber: restarting dose escalation may reduce gastrointestinal symptoms. Confirm instructions against your exact product label.

Missed weekly tirzepatide (Mounjaro)

For Mounjaro, prescribing information commonly advises taking the missed dose within 4 days (96 hours). If more than 4 days have passed, skip it and take the next dose on the regularly scheduled day. Leave at least 3 days (72 hours) between two doses when changing the dosing day.

Why you should not double the next dose

Taking extra medicine to catch up can increase nausea, vomiting, dehydration and other adverse effects. Never take two weekly doses together. If you are unsure when you last injected, verify your diary or pen record and contact your clinician.

What if you missed several weeks?

After a prolonged interruption, tolerability may change. Contact your prescriber before restarting at a higher dose, especially if you previously had significant nausea, poor oral intake or dehydration. A lower restart dose or renewed escalation may be appropriate depending on the medicine and situation.

What happens to blood sugar or appetite?

Appetite may increase and glucose control may worsen during interruptions. If you have diabetes, monitor glucose as advised and do not change insulin or sulfonylurea doses without a personalized plan. Persistent high glucose or symptoms of dehydration warrant medical review.

Travel and storage planning

Use a reminder and keep a record of injection dates. Follow your product’s temperature and storage instructions, which differ by brand and pen. Our GLP-1 travel guide covers planning for journeys.

When to seek medical advice urgently

Seek urgent assessment for severe persistent abdominal pain, repeated vomiting with inability to keep fluids down, fainting or signs of severe dehydration. For severe low glucose in a patient taking other glucose-lowering medicines, follow an established hypoglycaemia plan and obtain emergency help as appropriate.
